Please read the section on Grade Challenges in the Graduate Student Handbook before filling out this form.

This form is to be used when filing a grade challenge. Please read it carefully and provide all the requested information. Once you have completed the form and gathered the necessary documentation to support your challenge, please submit a single PDF file containing your form and materials to Erica Godfrey in the LAS Dean's Office for distribution to the LAS Grade Challenge Committee.
